How do health systems translate understanding about the social determinants of health into action? ProMedica, a nonprofit health system serving 27 counties in northwest Ohio and southeast Michigan, runs several community programs to address social determinants of health (SDOH). ProMedica currently screens patients to determine if they have food, housing and other basic needs, has pioneered a food pharmacy (prescribing food as medicine), and invested in inner-city grocery stores with healthy food.
