IMPROVING CARE OF SERIOUS AND LIFE-LIMITING ILLNESS

Many of us will experience serious illness during our lifetimes, and will need person-centered support and palliative care for ourselves and our caregivers. We also want to know that our wishes and goals of care at the end of life will be honored. GIA offers unique opportunities for funders to learn and work together to champion and improve serious illness and end-of-life care.

Serious Illness & End-of-Life Funders Community

The Serious Illness & End-of-Life Funders Community provides a forum for funders with a shared commitment to improving the care of people with serious illness and their families and caregivers. Launched in 2015, the Community now includes more than 30 funders.

To mark the fifth anniversary of the Serious Illness & End-of-Life Funders Community in 2021, GIA released this collection of stories and reflections shared by members of the group. You'll read about key developments in the field and about the power of networking as funders describe learning, drawing inspiration from each other, forging new partnerships, and catalyzing this dynamic young field together. 

Creating New Connections: How Philanthropy Can Support Better Care for People with Complex Health and Social Needs

People with complex health and social needs such as chronic conditions, behavioral health issues, food insecurity, trauma, and experiencing homelessness often encounter a healthcare system that is poorly coordinated, inefficient, expensive, and does not meet their needs. This guide assists funders of all types and sizes in finding opportunities in the complex care field and seeing their existing projects through a complex care lens.
